Case Capsule - Large Hydatid Cyst Liver with Obstructive Jaundice

A 35-year-old gentleman presented with jaundice and upper abdominal pain of 2 weeks duration, associated with fever since 2 days.

Blood investigations showed obstructive jaundice. CT Abdomen and pelvis with MRCP confirmed a large hydatid cyst in the right lobe of the liver measuring 12x14 cm, compressing the right biliary ducts (posterior ducts), with no obvious biliary communication. Associated cholecystitis with sludge in the gallbladder was also noted.

Medical Gastroenterologist (Dr. Chandrakanth) opinion was taken, and the patient underwent ERCP with stent placement into the right system.

The patient was started on Albendazole.

After 4 weeks of Albendazole, we performed Laparoscopic Hydatid cyst excision with lavage, drain placement, and Cholecystectomy.

1x10 mm and 3x5 mm ports were used for the surgery.

The patient was mobilised on Day 0 and discharged on Post-op Day 4, and continued on Albendazole. The abdominal drain was removed on Day 7.

At follow-up, the patient had recovered well and returned to his routine.
