Our First Robotic Bariatric Surgery in a Super Obese patient with Metabolic Syndrome

A 38-year-old gentleman with a BMI of 51.5 underwent Robotic Roux-en-Y Gastric Bypass (RYGB) surgery at Fortis Cunningham. The patient, who had been struggling with progressive weight gain and metabolic syndrome, had not responded to diet, exercise, or medical management, including GLP-1 analogue Semaglutide.

Pre-Surgery Comorbidities
  • Hypothyroidism
  • Hypertension
  • Dyslipidemia
  • Type II Diabetes Mellitus
  • Obstructive Sleep Apnea (OSA) on CPAP
  • Bilateral varicose veins
  • Limited mobility (unable to walk more than half a kilometer)
Surgical Outcome

The patient was mobilized within hours of extubation and started on sips of water from Day One. He was discharged on Post-op day 3 on a liquid diet. Notably, his blood sugars and pressures normalized, and he was off oral hypoglycemic agents (OHA) at discharge.

Collaborative Multidisciplinary Care

The successful outcome is attributed to the expertise of various departments involved in his care, including:

  • Anesthesia
  • OT staff
  • ICU
  • Pulmonology
  • Cardiology
  • Internal Medicine
  • Dietitian
  • Physiotherapy
  • Nursing staff
Conclusion

This case highlights the effectiveness of robotic bariatric surgery in managing super obesity and metabolic syndrome. The patient's rapid recovery and improved health outcomes demonstrate the benefits of this advanced surgical approach.
